The practical upshot: anything that is actually licensed to treat Malassezia in dogs is POM-V, so the vet visit is not optional for a first episode. Step 1: the vet visit — what to expect

The World Association for Veterinary Dermatology consensus guidelines — whose lead author is Professor Ross Bond of the Royal Veterinary College — rate twice-weekly bathing with a 2% miconazole / 2% chlorhexidine shampoo as the topical treatment with the strongest evidence for Malassezia dermatitis in dogs. In the UK that product is Malaseb (Dechra), a POM-V medicine. The usual pattern is a bath twice a week with about ten minutes of lather contact before rinsing, for at least three weeks, then a recheck. Yeast otitis is treated with prescription ear preparations that combine an antifungal with an anti-inflammatory and usually an antibiotic. Examples authorised in the UK include Surolan (Elanco; miconazole, polymyxin B and prednisolone), Easotic (Virbac; miconazole, gentamicin and hydrocortisone aceponate) and Osurnia (terbinafine, florfenicol and betamethasone, indicated for otitis externa associated with Staphylococcus pseudintermedius and Malassezia pachydermatis). For widespread, severe or shampoo-resistant cases — or a dog who simply cannot be bathed twice a week — vets add an oral azole antifungal, most often ketoconazole or itraconazole. The WAVD guidelines rate their supporting evidence in dogs as moderate. The cascade, explained for owners

The cascade is the part of the Veterinary Medicines Regulations that lets a vet prescribe outside a medicine's licence when, in the VMD's words, there is no available veterinary medicine authorised for the specific indication or condition in the animal being treated. In Great Britain the steps run in order: a UK veterinary medicine authorised for a different species or condition; then a UK-authorised human medicine or a veterinary medicine authorised outside the UK; then a medicine specially prepared (extemporaneous) by a vet, pharmacist or authorised manufacturer.
